If you would like to make an appointment with Dr Courtney you may complete and submit this form and we will contact you to confirm the time. If your appointment is of an urgent nature then please phone our rooms directly between 8am and 5pm QLD, phone 07 5630 2600. If you do not hear from us within two business days please give us a call.

Please include patient name, phone numbers, address, DOB, HF name and number, referral Doctor and any comments with regard your preferred appointment time.  Please note our appointments take place at John Flynn Hospital in Tugun.  To claim a medicare rebate for some of your appointment cost you will need a referral from your GP.
